This is a sample letter to a Civil Registration Office in Poland. The contents are translated using simple text with additional comments shown in italics.

 I would like to obtain a copy of the birth certificate, or if that's not possible an abbreviated birth certificate, for the following person:

Words to be substituted for "birth" in the letter (death, etc,) are provided.

Defining your relationship to the person whose certificate you are applying for is required. Additional words of kinship in Polish are: ojciec (father), matka (mother), babka (grandmother), prababka (greatgrandmother), dziadek (grandfather), pradziadek (greatgrandfather).

Please pay strict attention to the Polish diacritical marks!
